Parental Release of Educational Records


RE: Parental Release of School Records of to the Gallini Group.


I, , give my permission to release the master school file of my child, , to the Gallini Group.

34 C.F.R. §300.501 provides that the parents of a child with a disability must be afforded, in accordance with the procedures of §300.562-300.569, an opportunity to: (1) Inspect and review all education records with respect to:(i)       the identification, evaluation and educational placement of the child; and (ii) the provision of FAPE to the child.

34 C.F.R. §300.562 states that each participating agency shall permit parents to inspect and review any education records relating to their child that are collected, maintained, or used by the agency under this part. The agency shall comply with request without unnecessary delay and before any meetings regarding an IEP, or any hearing…and in no case more than 45 days after the request has been made.

The right to inspect and review education records under this section includes:

  1. the right to a response from the participating agency to reasonable requests for explanations and interpretations of the records
  2. the right to request of the agency provide copies of the records containing the affirmation if failure to provide those copies would effectively prevent the parent from exercising the right to inspect and review the records
  3. the right to have a representative of the parent inspect and review the records.

So, the prior request made is for a copy of the following documents:

  1. Report cards or progress reports
  2. Transcripts
  3. Notices of any unsatisfactory performance, however labeled.
  4. All results of any group administered tests.
  5. Attendance records, including any notices related to attendance problems.
  6. Any records of discipline.
  7. All communications requesting Individualized Education Program (IEP) meetings.
  8. All communications setting IEP meetings, parent-teacher conferences, or any communication between District personnel and the family related to the student.
  9. All IEP documents of any type whatsoever
  10. All documents relating to the student’s progress in meeting the IEP goals and objectives.
  11. All medical and other school health records of any type.
  12. All correspondence, memorandum, and notes relating to the student.
  13. All notes of any telephone calls relating to the student.
  14. Copies of all assessments administered by the District personnel at any time.
  15. All notes from any school employee regarding the provision of services to the student, including copies of all records showing the dates and times on which the student was provided with any related service.
  17. Any emails or copies of electronic transfers that concern the student.
  18. Any and all tape or video recordings with regard to the child.
  19. EVERYTHING and ANYTHING pertaining to this child.
